alarmtimer: Use hrtimer per-alarm instead of per-base

Arve Hjønnevåg reported numerous crashes from the
"BUG_ON(timer->state != HRTIMER_STATE_CALLBACK)" check
in __run_hrtimer after it called alarmtimer_fired.

It ends up the alarmtimer code was not properly handling
possible failures of hrtimer_try_to_cancel, and because
these faulres occur when the underlying base hrtimer is
being run, this limits the ability to properly handle
modifications to any alarmtimers on that base.

Because much of the logic duplicates the hrtimer logic,
it seems that we might as well have a per-alarmtimer
hrtimer, and avoid the extra complextity of trying to
multiplex many alarmtimers off of one hrtimer.

Thus this patch moves the hrtimer to the alarm structure
and simplifies the management logic.

Move ~TEGRA_DC_OUT_INITIALIZED_MODE mask to
end of dc probe. This is fine since panel enable in the
first kernel boot will be still skipped, but in the
next panel enable like resume, it will be operated correctly.
With this flag, we skip cmu init in the first
kernel boot to avoid color tone change by cmu_init.
Once probe is done, cmu change in sysfs node from
user space will be applied and cmu will be enabled
or disabled properly.

Now, nvidia,cmu-enable property for dc0/dc1 are
set to 1 for all tegranote lines.

 - Reduce fb2 size to SZ_2M from SZ_16M

 - In normal kernel mode, single buffer is used
   for fb1. Reduce fb1 size to (SZ_4M + SZ_512K)
   from (SZ_16M + SZ_2M) for TN7C, from (SZ_8M + SZ_1M)
   for TN7.

 - Prepare helper function to know if current kernel
   mode is normal one or recovery one because
   minui needs 2 X fb_size because of double buffer.

2. Verified
 - Seamless display transition still works.
 - fb2 mode change in HDMI by FBIOPUT_VSCREENINFO,
   FBIOGET_VSCREENINFO works.
 - recovery screen is fine.
 - charger screen is fine.

3. Todo
 - Need to find the reason why smaller fb2 size
   matters in minui.

add extra free kbytes tunable

Add a userspace visible knob to tell the VM to keep an extra amount
of memory free, by increasing the gap between each zone's min and
low watermarks.

This is useful for realtime applications that call system
calls and have a bound on the number of allocations that happen
in any short time period.  In this application, extra_free_kbytes
would be left at an amount equal to or larger than than the
maximum number of allocations that happen in any burst.

It may also be useful to reduce the memory use of virtual
machines (temporarily?), in a way that does not cause memory
fragmentation like ballooning does.

Revived for use on old kernels where no other solution exists.
The tunable will be removed on kernels that do better at avoiding
direct reclaim.

lowmemorykiller: make default lowmemorykiller debug message useful

lowmemorykiller debug messages are inscrutable and mostly useful
for debugging the lowmemorykiller, not explaining why a process
was killed.  Make the messages more useful by prefixing them
with "lowmemorykiller: " and explaining in more readable terms
what was killed, who it was killed for, and why it was killed.

The messages now look like:
[   76.997631] lowmemorykiller: Killing 'droid.gallery3d' (2172), adj 1000,
[   76.997635]    to free 27436kB on behalf of 'kswapd0' (29) because
[   76.997638]    cache 122624kB is below limit 122880kB for oom_score_adj 1000
[   76.997641]    Free memory is -53356kB above reserved

A negative number for free memory above reserved means some of the
reserved memory has been used and is being regenerated by kswapd,
which is likely what called the shrinkers.

tcp: add a sysctl to config the tcp_default_init_rwnd

The default initial rwnd is hardcoded to 10.

Now we allow it to be controlled via
  /proc/sys/net/ipv4/tcp_default_init_rwnd
which limits the values from 3 to 100

This is somewhat needed because ipv6 routes are
autoconfigured by the kernel.

input-cfboost: use RT kthread for PM QoS request

It has been observed that input drivers worker thread gets scheduled
before boost worker thread, specifically in case of Raydium touch.
The delayed scheduling of boost works adds to first touch latency
when cpu is idle and running at lower clock rate. Hence use RT
kthread for fast scheduling of boost work. Thread Priority is set to
MAX_RT_PRIO-10 so that the highest RT priority level is left for
EMC DVFS.

If uaddr == uaddr2, then we have broken the rule of only requeueing from
a non-pi futex to a pi futex with this call.  If we attempt this, then
dangling pointers may be left for rt_waiter resulting in an exploitable
condition.

This change brings futex_requeue() in line with futex_wait_requeue_pi()
which performs the same check as per commit 6f7b0a2a5c0f ("futex: Forbid
uaddr == uaddr2 in futex_wait_requeue_pi()")

[ tglx: Compare the resulting keys as well, as uaddrs might be
   different depending on the mapping ]

Fixes CVE-2014-3153.

When trying to delete a table >= 256 using iproute2 the local table
will be deleted.
The table id is specified as a netlink attribute when it needs more then
8 bits and iproute2 then sets the table field to RT_TABLE_UNSPEC (0).
Preconditions to matching the table id in the rule delete code
doesn't seem to take the "table id in netlink attribute" into condition
so the frh_get_table helper function never gets to do its job when
matching against current rule.
Use the helper function twice instead of peaking at the table value directly.

net: support marking accepting TCP sockets

When using mark-based routing, sockets returned from accept()
may need to be marked differently depending on the incoming
connection request.

This is the case, for example, if different socket marks identify
different networks: a listening socket may want to accept
connections from all networks, but each connection should be
marked with the network that the request came in on, so that
subsequent packets are sent on the correct network.

This patch adds a sysctl to mark TCP sockets based on the fwmark
of the incoming SYN packet. If enabled, and an unmarked socket
receives a SYN, then the SYN packet's fwmark is written to the
connection's inet_request_sock, and later written back to the
accepted socket when the connection is established.  If the
socket already has a nonzero mark, then the behaviour is the same
as it is today, i.e., the listening socket's fwmark is used.

Black-box tested using user-mode linux:

- IPv4/IPv6 SYN+ACK, FIN, etc. packets are routed based on the
  mark of the incoming SYN packet.
- The socket returned by accept() is marked with the mark of the
  incoming SYN packet.
- Tested with syncookies=1 and syncookies=2.

Kernel-originated IP packets that have no user socket associated
with them (e.g., ICMP errors and echo replies, TCP RSTs, etc.)
are emitted with a mark of zero. Add a sysctl to make them have
the same mark as the packet they are replying to.

This allows an administrator that wishes to do so to use
mark-based routing, firewalling, etc. for these replies by
marking the original packets inbound.

Tested using user-mode linux:
 - ICMP/ICMPv6 echo replies and errors.
 - TCP RST packets (IPv4 and IPv6).

Currently, IPv6 router discovery always puts routes into
RT6_TABLE_MAIN. This causes problems for connection managers
that want to support multiple simultaneous network connections
and want control over which one is used by default (e.g., wifi
and wired).

To work around this connection managers typically take the routes
they prefer and copy them to static routes with low metrics in
the main table. This puts the burden on the connection manager
to watch netlink to see if the routes have changed, delete the
routes when their lifetime expires, etc.

Instead, this patch adds a per-interface sysctl to have the
kernel put autoconf routes into different tables. This allows
each interface to have its own autoconf table, and choosing the
default interface (or using different interfaces at the same
time for different types of traffic) can be done using
appropriate ip rules.

The sysctl behaves as follows:

- = 0: default. Put routes into RT6_TABLE_MAIN as before.
- > 0: manual. Put routes into the specified table.
- < 0: automatic. Add the absolute value of the sysctl to the
       device's ifindex, and use that table.

The automatic mode is most useful in conjunction with
net.ipv6.conf.default.accept_ra_rt_table. A connection manager
or distribution could set it to, say, -100 on boot, and
thereafter just use IP rules.

Used common callbacks for shared bus cap and floor sysfs show/store
operations, and PM QoS notifier. Only final bus user update operation
is different, since applying floor implies that shared bus should be
enabled, while cap does not require it.

As a result of this refactoring PM QoS optional support implemented
for bus caps become available for bus floors as well.

In order to check smps10_out1 enabling, read smps10_ctrl instead of
smps10_status.

In factory line, few board has no smps10_out1 output.
dc driver definitely enable smps10_out1 during booting and
disable/re-enable at suspend/resume.
No repro in SQA side and no chance to get the additional log
to identify this issue.
So, apply this change for the possible cause of it because
smps10_out1 will not be enabled if smps10_status reports true.

video: tegra: dc: fix nvsd smooth_k handling

When smooth_k is enabled, the NVSD driver determines
that hardware pixel gain and brightness calculations
are complete when it sees to consecutive and equal
brightness values. However, since multiple raw K
values are mapped to a single brightness value,
this check can be misleading.

To fix this, we continue checking for new brightness
values for a few additional frames after seeing repeating
brightness results. The number of frames is calculated
from smooth_k_incr.
